Accident and incident investigations
Find the causes. Set the actions.
When something goes wrong you need to know why, and what to change. We carry out structured investigations that get past blame to the real causes.
Emergency first. If someone is hurt or there is immediate danger, deal with the emergency and make the area safe first. Call 999 if needed. Some incidents must also be reported to HSE under RIDDOR by the responsible person. This website is not an emergency service. What must be reported, and when.

How it works
Four simple steps.
- 01
Contact
A short description of the type of incident is enough to start. We agree the scope, and how evidence and personal details are handled.
- 02
Gather
Evidence, documents and conversations with the people involved.
- 03
Analyse
Immediate causes, underlying causes and what allowed them.
- 04
Report
Findings, recommendations and an action plan you can track.
Good to know
- Reporting under RIDDOR stays with the responsible person, usually the employer.
- We are not an emergency service. Deal with the emergency and make the area safe first.
How it is priced
Quoted to scope
Shaped by the incident, the evidence to review, the people to speak to and the report needed. You get a short written proposal before anything starts.
Common questions
Is this the same as reporting to HSE under RIDDOR?
No. Some incidents must be reported to HSE under RIDDOR by the responsible person, and that duty stays with you. An investigation is about understanding causes and preventing a repeat. We can point you to the right reporting route.
How quickly can you start?
Contact us as soon as you can; evidence and memories fade quickly. We agree timescales when we scope the work.
Is the investigation confidential?
Yes. We agree who receives the report and how sensitive information is handled before we start.
